Framework for Early Detection and Diagnosis of Alzheimer’s Disease

This tool helps primary care and specialty care teams work together. It provides step-by-step workflows, clear roles for each team member, and ways to coordinate care. This tool is for primary care teams (doctors, nurse practitioners, physician assistants, social workers, and nurses) and specialists.

Informed by Expertise and Lived Experience

UsAgainstAlzheimer's created these resources with help from two advisory groups. The groups shared feedback through monthly meetings, one-on-one talks, email, and document reviews. The advisory groups included healthcare professionals, people with personal experience with memory changes, and caregivers for people experiencing memory changes. Their input shaped these tools. The advisory groups included:

  • Lived experience advisory group: People living with mild cognitive impairment due to Alzheimer’s disease, people living with increased risk of Alzheimer’s disease (APOE e4 carriers), current and former caregivers to family members with Alzheimer’s disease, licensed clinical social workers, etc.

  • Expert advisory group: dementia experts, general neurologists, primary care providers, specialists, researchers, licensed clinical social workers, and nurses.

UsAgainstAlzheimer’s is a nonprofit patient advocacy organization which aims to catalyze change in the health ecosystem to improve dementia prevention, early detection, and access to treatments through advocacy, policy, convening, and high-leveraged programs, with a particular focus on health equity.
